What is plantar fasciitis/fasciopathy?
Plantar fasciopathy (also known as plantar fasciitis or heel pain) is one of the most common foot complaints around. It involves increased loading of the plantar fascia, which is a band of tissue that runs along the underside of the foot, connecting your heel to your toes. Despite the name, inflammation isn't always present, which is why "fasciopathy" is now the preferred term.
The most common sign is pain at the bottom of the heel, often worst with the first steps in the morning or after sitting for a while, then easing as you get moving.
What causes plantar fasciitis/fasciopathy?
Plantar fasciopathy usually develops when the plantar fascia is repeatedly asked to handle more load than it can manage. Contributing factors include:
- Prolonged standing or walking: Spending long periods on your feet, particularly on hard surfaces, increases cumulative load through the plantar fascia.
- Sudden increases in activity: Ramping up exercise volume or intensity too quickly without time to adapt is a common trigger.
- Footwear: Shoes with a narrow toe box prevent the foot from splaying naturally and when combined with a stiff sole, restrict the muscles ability to take on load, putting more load onto the plantarfascia.
- Weakness in the foot, ankle, and hip: When the muscles that support the foot and lower limb aren't strong enough, more stress falls on the plantar fascia.
- Reduced ankle mobility: Less range of motion at the ankle increases tension through the plantar fascia with every step.
- Movement patterns at the knee and hip: How the whole lower limb moves influences how load is distributed through the foot.
- Metabolic factors: Conditions such as diabetes and obesity are associated with higher rates of plantar fasciopathy.
What to do
- Track changes in activity and capacity: Tracking what increases and decreases symptoms of dysfunction gives you useful information about what your foot is and isn't ready for yet.
- This is a load management issue: The plantar fascia has been asked to handle more than it's currently conditioned for. Recovery isn't about resting it completely; it's about finding the right level of activity and building capacity from there.
- Passive treatments have limits: Massage, stretching, and orthotics can provide effective but often only temporary relief. They work best as part of a broader plan that includes building strength and improving foot function.
- Review your footwear: Transitioning to shoes with a wider toe box and more flexible sole can meaningfully reduce load through the plantar fascia. If you're in significant pain, make this change in footwear thoughtfully as it is often one of the most impactful things you can do
- Keep moving carefully: Complete rest is rarely the right answer. Modifying what aggravates it while staying active tends to produce better outcomes
- Address ankle mobility and foot strength: These are often significant contributors and are very trainable with the right approach. Additionally, hip strength, and movement patterns further up the chain all influence how much demand lands on the plantar fascia. Addressing just the heel often isn't enough.
- Work with a trained professional: A podiatrist or physiotherapist can identify the specific contributors for you and build a plan that fits your activity level and circumstances

Mobilisation: There are 33 joints in each foot, and joints exist for movement. Modern footwear and inactive environments restrict many of them. Targeted mobility work restores the range of motion the foot was built to express.
Coordination: Range of motion needs to be controlled to be useful. Coordination is the foot responding to load and varied surfaces automatically, without conscious effort. It starts with focused practice and becomes subconscious over time.
Integration: The foot does not work in isolation. Every step integrates it with the ankle, knee, hip, and the rest of the body. Building foot function means working toward that full integration, not just isolated strength or flexibility.
